I've come across it in a few different types of fic - and it drives me nuts. If the fic's otherwise good enough that I'm actually reading it in the first place, then I'll keep reading, but there have been a few times that I've just gone to find something else to read... I've found it in several genres of fic, but usually only at fanfiction.net...
